Our Services
We provide the following services:
- Assessment:
- We look at all aspects of our clients’ physical, mental, emotional, financial, and legal situation to develop a comprehensive care plan.
- Care Management:
- If requested, we work in conjunction with the individual and/or family to carry out the care plan.
- We coordinate services with physicians, health care professionals, attorneys, trust officers, accountants, guardians, POA’s and other involved individuals to form a comprehensive care team that will most effectively serve the needs of the individual and his or her family.
- Crisis Intervention
- If necessary, we are ready to react to and help manage a crisis situation.
- Insurance and Entitlements:
- We help cut through red tape and bureaucracy to assure that our clients secure the private and public benefits to which they are entitled.
- Education and Counseling:
- Our aim is to assist individuals and their families manage their care as independently as possible. Therefore we provide the information and support to enable our clients to manage as much of their care as they choose.
- Information and Referral:
- Our familiarity with community resources saves our clients hours of time and points them to the information and services which will most effectively help them meet the challenges they face.
- Placement
- Our first goal is to help individuals remain in their own homes. In order to facilitate this, we help arrange for assistance with bill paying, yard work, meal preparation, home modifications, in-home nursing care, companion living or day care--in short, anything it takes to help the elder or disabled person function at home.
- When an individual is living with a family member, we help arrange for respite care or any other services that will help the caregiver function more effectively.
- We recognize the need for placement in retirement facilities, assisted living facilities, and nursing homes. We guide our clients to find the right community, assist them in successfully negotiating arrangements for these services, and continue to provide care management as requested to assure optimal functioning.
- Advocacy
- We realize that older individuals, persons with disabilities and their families frequently need help in dealing with those responsible for their care. We provide advocacy in all settings to insure our clients’ rights and to secure optimal treatment.
